공공서비스 공급에 있어 정부냐 시장이냐 하는 논쟁은 거버넌스에 대한 논의가 전개되면서 이제 경쟁과 협력의 새로운 방식에 대해 관심을 집중하게 되었다. 대체로 이러한 방식은 정부와 시장, 그리고 시민사회가 상호 협력적 네트워크를 통해 서비스전달과정에 있어 시너지 효과를 도모한다는 것이다. 복지혼합의 관점에서 볼 때 우리의 경우 정도의 차이가 있을지라도 이미 사회복지서비스 전달과정에 민간의 역할 비중은 정부의 그것을 훨씬 앞지르고 있다. 따라서 노인복지서비스처럼 복잡한 욕구에 대응하기 위한 수단은 네트워크와 같은 복합기제가 타당하리라는 주장이 설득력을 얻고 있는 이때, 공공과 민간공급자간 연계관계를 탐색한다는 것은 네트워크전달체계를 구축하기 위해 매우 유용한 작업일 것이다. 이러한 인식을 배경으로 본 연구는 서울시 노인방문(가정)간호서비스를 사례로 하여 서비스 전달과정에 참여하는 행위자를 정부(보건소), 시장(가정간호시범병원), 시민사회(간호사회)의 관점에서 포착하여 이들간의 플라이언트 의뢰 네트워크를 탐색하였다. 이는 많은 선행연구들이 제기하는 방문(가정)간호와 재가복지간의 효과적 연계를 도모하기 위한 전제로서 탐색적인 것이다. 분석결과 간호사회는 대부분의 참여자들로부터 주로 클라이언트를 의뢰받고 있으며, 가정간호시범병원은 병원간 연계활동은 활발하나 여타 참여자와의 연계관계는 미약한 것으로 보인다. 보건소는 가정간호시범병원과 간호사회간 클라이언트를 의뢰하고 받는 매개적 위치에 있는 것으로 분석된다. 따라서 재가복지 체계와의 연계를 위해서는 보건소의 매개적 역할을 중심으로 네트워크 설계가 이루어져야 할 것이며, 간호사회와 같은 다양한 시민사회 자원을 축적할 수 있는 정책적 노력이 이루어져야 할 것이다.
Governance paradigm has been spread over nations for social welfare service provision as distinguished from traditional bureaucratic model. This study examines the basic conditions for building the social welfare service network through exploring the structure of community care for old age service delivery network in which government, market, and NGO interact. The term `network` is regarded as problem-solving mechanism and implementation structure of governance. The definition of community care for old age is operational concept that integrates home based service composed of day care, short stay, home helper service and home nursing care. The reason of using this kind of concept is, first of all, that health and medical services are important for old age care according to the results of existing research. And it is easy to explore the relationships of government, market, and civil society in community care delivery system. Case study was conducted to figure out the specific network structure focused on client referral network. According to the result, health care office has central position in client network. Inter-organizational network activity among home-based service, organization, hospital-based home nursing care, visiting nursing office is limited, and especially community-based home nursing service which has been provided by Seoul Nurse Association stopped through legal amendment. It is necessary to facilitate accumulation of resources that activate community care network for old age.
