본 논문은 2000년 7월 이후 통합 실시되고 있는 국민건강보험에서 계층간의 소득재분배효과(계층간 보험료 이전효과)를 분석하였다. 분석 결과, 보험 급여율에 있어서는 지역가입자나 직장가입자나 모두 수혜효과를 누리고 있으며 전반적으로 소득재분배 즉, 보험료 이전효과가 있다고 판단된다. 그러나 보험료 점유율과 보험 급여비 점유율에 있어서는, 평균보험료는 직장 가입자의 보험료가 상대적으로 적으나, 보험 급여비는 직장가입자의 수혜 정도가 지역가입자보다 상대적으로 높다. 보험료 점유율에 있어서는 저소득에서는 직장가입자가 직역가입자보다 상대적으로 더 높고, 고소득층에서는 지역가입자의 점유율이 상대적으로 더 높다. 보험 급여비 점유율에 있어서는, 저소득에서는 직장가입자가 상대적으로 더 높고, 고소득층에서는 지역가입자의 보험 급여비 점유율이 상대적으로 더 높다. 소득 이전율에 있어서는 지역가입자나 직장가입자 모두 건겅보험에 의하여 소득재분배 즉, 소득이전 효과가 있는 것으로 판단되었다. 10분위 분배율에 있어서는 보험료의 경우는 직역가입자나 직장가입자 모두 어느 정동의 불평등은 있다고 판단되며, 보험 급여비의 경우는 직역가입자나 직장가입자 모두 불평등도는 거의 없다고 판단되었다. 불평등도에 있어서는 직역가입자나 직장가입자나 모두 불평등성이 없는 것으로 보이지만 양 집단간에는 직역가입자가 직장가입자보다 상대적으로 더 불평등적인 것으로 판단되었다.

Effect of income-redistribution is analyzed in this paper on the basis of individual insurance contribution in merge health insurance program of Korea since July 2000. Rate of merged health insurance benefit by class showed almost all the insurees have the effect of income redistribution. Average insurance contribution of job-insurees is lower than that of area-insurees, but average insurance benefit of job-insurees is higher than that of area-insurees. Income-transfer rate of medical insurance program showed the effect of income redistribution was improved for all the insurees. Decile distribution ratio on the insurance contribution showed some degree of inequality, but it almost didn`t show inequality on the insurance benefit. In inequality or equity, it showed that there is more inequality in job-insurees than in area-insurees . According to the results of the analysis, it could be said that there is the effect of income redistribution in merged health insurance system, but it should be reconsidered in imposing health insurance contribution.
