2013년 보건복지부가 발표한 간호인력 개편 계획에는 실무간호사(Licensed Practical Nurse, LPN) 양성 계획이 포함되어있다. 간호사(Registered Nurse, RN) 부족이 극심한 의료기관에서 LPN을 이용한 인력 대체(skill mix)가 나타날 것으로 예상된다. 본 연구는 LPN 제도를 시행해온 미국의 자료를 이용한 간호사 부족과 RN-LPN 간 인력대체 사이의 상관관계 분석을 목적으로 하며, 간호인력 수급 관련 논의에 실증적 근거를 제공하고자 한다. 300여개 급성기 병원의 10년간 고용 자료를 이용, 패널자료분석법을 적용한 종단연구를 수행하였다. 분석 결과 LPN 수요 증가와 간호사 부족 간 상관관계를 확인할 수 없었다. 반면 RN 임금 수준과 병원 특성 및 의료 환경 특성과 밀접한 상관관계를 나타내었다(RN 임금 변동에 따른 탄력성: 0.70, p=.094; 입원환자 증가에 따른 탄력성: 0.89, p<.001; 메디케이드 및 메디케어 환자 증가에 따른 탄력성: 0.04, p<.001). 이는 간호사 부족 보다는 노동 비용 절감을 위한 전략적 선택이 LPN 수요증가의 주요 원인이라는 의미이다. 캘리포니아 주는 미국 내에서 LPN 이용이 비교적 활성화된 곳으로 알려있는데, 이를 감안할 때 연구의 결과를 일반화하기 어렵다는 지적이 있다. 이에 본 연구는 의료기관의 고유특성의 영향과 노동 수요의 임금 내생성 문제를 가능한 통제하고자 하였다. 본 연구의 결과를 바탕으로 재정 상태가 열악하여 인건비 절감 유인이 강한 의료기관에서 인력대체 수요가 높을 것으로 예상해 볼 수 있다.

This study examines what factors are related to hospital demand for Licensed Practical Nurses (LPN) and how the demand is changing during periods of nursing shortages. Using hospital-level data from California`s Office of Statewide Health Planning and Development, we estimate the demand for LPN. Several statistical methods are applied to control for a variety of possible factors related to hospital decisions regarding employment. We found that the demand for hospital LPN rose with increases in wages of RN, where increases in RN wages by 10.0 percent were related to increases in the demand for LPN by about 7.0 percent. Factors found to be related to the demand include patient days, patient day fluctuation, and the level of financial pressures. We did not find a significant trend of increasing demand for LPN during the years of RN shortage. In conclusions, it is indeterminable that LPN is a potential worker to figure out hospital nursing shortages. Although our findings show in part that LPN may substitute hospital RN, further study on potential determinants of substitutions such as scope of practice is needed.
