본 연구는 2008년 노인장기요양제도 출범이후 나타나는 제도의 실질적 사각지대를 규명하고 해소 방안들을 논의하기 위해 실행되었다. 장기요양보험제도 자체의 문제점에 대한 비판들에 대한 선행연구가 증가하는 추세에 있으나 실제 정확한 사각지대 규명과 규모추정, 발생원인 등에 대한 연구는 전무하다. 본 연구에서는 기존의 관련 문헌자료들을 정리하여 다양한 사각지대 발생에 대한 시각들을 논의하였다. 특히 사각지대를 대상포괄성의 역의 개념으로 접근하여 실질포괄성의 개념을 적용하였다. 연구결과 사각지대는 대상자선정과 관련하여 평가판정기준과 소득에 의해 1차적으로 나타났고, 공적부조에서 사회보험의 전환에 따른 본인부담 등에 의한 경제적 욕구에 의해서도 발생되는 것으로 나타났다. 또한 저소득층 등급외자에 대한 국내 현행 돌봄 사업의 문제점을 짚어보고, 일본과 미국의 사례를 참조하여 저소득층 노인에 대한 사각지대 개선방안에 대해서도 제시해보았다. 주요 정책대안으로 치매특별등급도입에 따른 노인돌봄서비스의 내용보완, 등급외자 돌봄에 대한 국민건강보험공단과 지자체간의 연계, 연령기준 보완에 대한 정책적 개선방안들을 제시하였다.

In 2008, July, the NLTC(National Long-term Care for elders) policy for the elders was just started for expanding social services for elders to need long-term care regardless of their household incomes. During the last six years of implementation, however, new no-care zones were appeared in the service market due to the complex combination of eligibility and delivery systems of NLTC and social care services for elders. The purpose of this study is to analyze critical policy issues around the LTC policy and social care for elders and to identify the No-Care Zone of the policy. The key contents of this study were as follows. First, the study identified no-care zones around LTC for the elders by distinguishing several zones by LTC care levels and incomes. Secondly, the study discussed some no-care zone issues raised by the transition from tax-based funding to social insurance funding for the low-income no-eligible population groups(LTC level 4 and 5). In third, the study discussed the current care policies of Korea, Japan and USA specially for low-income elders. In fourth, the study suggested some policy changes of social care specially for elders with. Finally, the study suggests several policy alternatives in order to reduce the size of no-care zones of NLTCI and social care services for elders.
