의원필담의 내용은 당시 병증(病症)과 치험례(治驗例), 물명(物名)이나 본초(本草) 관련 정체성 규명, 양국의 각종 풍속, 제도, 지리(地理) 등 다방면에걸쳐 정형화되어 있다. 그만큼 이는 당대 동아시아 풍속 및 문화지형도(文化地形圖) 구성상 그 가치가 크다. 통신사 의원필담 전반에 걸친 관심사를 종합하다 보면, 자연스럽게 18세기 조일 의료 풍속의 지형도가 그려지고, 그 안에 내포된 특성들이 돌출될 수 있으며, 이는 동아시아 풍속사의 구성 요소로 자리매김할 수 있다. 본 연구자는 우선 그 흔적을 ‘본초(本草)’와 ‘의료 분과의 특화(特化)’, ‘유행병(流行病)’이라는 세 부분에서 찾아보았고, 이를 통해 그 특성들을 확인해보는 데 본 논문의 목적이 있다. 본초를 중요하게 여기고 심도 있는 대화를 나누었던 이면에서 의서 중심의 의학적 지식보다는 실제 치료에 중점을 둔 그 당시 실용적 의료 풍속을 확인할 수 있다. 또한 의원필담의 주요 내용 중 하나는 치법과 관련된 내용인데, 그것은 실제 임상에 근거한 치법이라는 특성을 지닌다. 그리고 이는 조일 의원들 사이에 부인병과 소아병에 대한 특화된 관심의 확대로 표출된다. 아울러 의원필담을 남긴 일본 의원들의 주된 관심사 중 하나는 풍토병과 전염병 등 당시 일본에 유행하던 유행병들이었다. 광견병의 예에서 확인할 수 있듯이 전통 의학에 입각한 임상 처방을 중시하면서 치료에 힘쓰고자 했던 당시 조일 의료 풍속의 일반적 성향을 확인할 수 있다.
