본 연구는 한국 보건의료체계의 지난 10년간의 성과변화를 평가하고, 우리나라와 유사한 보건의료체계를 채택하고 있는 주요국가와의 비교를 통해 우리나라 보건의료체계의 위상을 진단하는 것을 목적으로 한다. 이를 위해 OECD Health Data2012에서 제공하는 보건의료자원과 재원조달, 보건의료활동, 건강수준 영역의 주요 지표를 선정하여 OECD 국가와 비교한 한국의상대적 위치를 파악하고, 우리나라와 유사한 간접제공 방식의 사회보험을 채택하고 있는 대표적인 선진국인 일본, 프랑스, 독일, 네덜란드 4개국과 비교하였다. 연구결과 한국은 병원, 병상, 고가의료장비의 과잉공급이 심화되고 있었으며, 진찰건수가 많고 재원일수가 길어 한국의 의료이용량은 일본을 제외하고 OECD 국가에서 가장 높은 수준이었다. 그리고 한국의 인구당 의료비와 GDP 대비 의료비의 상대위치는 아직까지 낮았지만 증가율은 높았으며, 기대수명, 조기사망, 영아사망률과 같은 객관적 신체적 건강성과의 상대수준이 크게 향상되었지만, 아직까지 건강수명 증가가 더디고, 인지된 건강수준은 낮은 상태로 정체되어 있었다. 이를 통해 한국의 보건의료체계 성과향상을 위해서 보건의료자원 투입의 적정성 유지를 위한 수급계획과 관리방안의 필요성과 과도한 의료이용을 방지하고 의료의 질을 제고하기 위한 방안이 필요함을 확인하였으며, 더불어 삶의 질을 포함한 전반적인 건강수준을 향상시킬 수 있는 방향으로 보건의료체계의 구조적 개선이 필요함을 알 수 있었다.
This study is aimed to analyze Korean health care system from 2000 to 2010 using 2012 OECD Health Database and to suggest policy recommendations based on these findings. For this purpose, the study identified the changes in Korean health care system in the aspects of resources, services and health status for the decade, and measured relative performance among OECD members by making comparison with four member countries - Japan, France, Germany, and Netherlands - for those countries have national health insurance(NHI) system similar to Korea. The results of this study can be summarized as follows: First, Korea over-supplied health care material resources such as hospitals, hospital beds and high price medical equipments, whereas under-supplied health care human resources including doctors and nurses. Second, Korea over-used health care services such as consultations with doctors and length of stay in hospitals. Third, While Korea attained dramatic improvement in health status such as increase in life expectancy and decrease in premature mortality and infant mortality, there are serious problems of mental and self-perceived health. This study found that the aspects of the change in Korean health care system is very similar to those of Japan. It is suggested to review strengths and weaknesses of Japanese health care system in order to respond various forthcoming changes in Korean health care system.
