목 적: 대퇴 전자하 골절은 금속정의 삽입이 원칙으로 알려져 있으나, 근위부 골편의 전위로 인해 이상적인 진입점을 찾기가 어렵다. 이에 저자들은 스테인만 핀을 이용하여 정복술을 시행하고 골수 내 금속정으로 내고정술을 시행한 경우의 치료 결과를 분석하여 이 수술적 술기의 유용성에 대해 알아보고자 하였다. 대상 및 방법: 2008년 1월부터 2013년 3월까지 대퇴 전자하 골절로 내원하여 폐쇄적 정복 및 골수 내 금속정으로 내고정술을 시행한 환자 중 1년 이상 추시 관찰이 가능하였던 33예를 대상으로 하였다. 수술 후 골유합 기간, 술 후 정복상태, 대퇴골경간각의 변화, 고관절 기능 평가, 합병증 등에 대해 알아보았다. 결 과: 스테인만 핀을 이용하여 정복술을 시행한 모든 경우에서 골유합이 이루어졌으나 지연유합이 3예 발생하였다. Fogagnolo 분류상 모두 acceptable 이상의 정복상태를 보였으며 대퇴골 경간각의 내반 변화는 0.94o±3.1o, Harris hip score는 술 전과 최종 추시 시 간에 유의한 차이를 보이지 않았다(p＞0.05). 결 론: 스테인만 핀을 이용하여 정복술을 시행하여 만족할만한 골유합 및 정복상태를 얻을 수 있었다. 따라서 스테인만 핀을 이용한 정복술은 대퇴 전자하 골절의 정복을 도와 수술을 쉽게 하는 유용한 술기라고 판단된다.
Purpose: Nail insertion is the treatment of choice for subtrochanteric femoral fracture, but displacement of proximal bone fragment makes it difficult to find an ideal entry point. Therefore, in this study we aimed to determine the usefulness of treatment of subtrochanteric femoral fracture using Steinmann pin assisted reduction, internal fixation, and insertion of intramedullary nails. Materials and Methods: We evaluated 33 patients who were followed-up more than a year with a displaced subtrochanteric femoral fracture treated with closed reduction and intramedullary nail fixation between January 2008 and March 2013. In addition, we studied postoperative bone union time, postoperative reduction status, change of the femur neck shaft angle, evaluation of hip joint function, return to daily life, and complications. Results: All fractures with Steinmann pin assisted reduction were united but they included three cases of delayed union. In Fogagnolo classification, all cases were up to acceptable states and the varus change of femur neck shaft angle was 0.94o±3.1o; no significant difference in Harris hip score was observed between preoperative and last follow-up (p＞0.05). Conclusion: There were satisfactory results in bone union and reduction state with Steinmann pin assisted reduction. Therefore, Steinmann pin assisted reduction is a useful surgical technique for subtrochanteric femoral fracture.
