본 연구는 상담 및 심리치료의 효과를 뇌영상법으로 검증한 최근 연구들의 주제와 방법을 검토함으로써 이 영역에서 뇌영상법의 가능성과 유의점을 제시하려는 목적으로 이루어졌다. 이를 위해 연구자는 1992년 1월부터 2014년 6월까지 상담 및 심리치료 영역에서 뇌영상법을 적용하여 국외 주요학술지에 발표된 총 46편의 경험논문을 수집하고 이에 대한 주제분석과 방법분석을 수행했다. 주제분석은 ‘무엇’에 해당하는 분석으로 연구자는 논문의 출간시기 외에 연구대상자 특성, 적용한 상담 및 심리치료 개입의 종류를 분석했다. 방법분석은 ‘어떻게’에 해당하는 분석으로 연구참여자 수, 개입기간, 통제 및 비교집단의 구성, 측정도구 및 실험조건의 특성, 척도의 추가적 사용 여부 등을 분석했다. 주제분석에서 연구대상자는 강박장애(23.9%), 개입 종류로는 인지행동치료 접근(63%)이 가장 많았다. 방법분석에서 연구의 통계력에 가장 큰 영향을 끼치는 참여자 수는 평균 24.6명이며 많은 연구들이 표본 수의 부족을 한계점으로 꼽았다. 개입기간은 평균 11.6주였고 54.6%의 연구가 대조군 으로 정상군 통제집단을 설정한 설계를 사용했다. 연구논문의 56.5%가 뇌영상법 중 기능적 자기공명영상(fMRI)을 사용했으며, 43.5%는 실험조건에서 휴지상태의 뇌활동을 측정했다. 또한 연구의 97.8%는 개입 이후 증상의 호전여부를 검증하기 위해 증상 개선정도를 별도로 측정했다. 변화기제규명과 효과적 개입방법 모색을 위한 상담 및 심리치료 연구에서 뇌영상법이 가진 장단점과 이를 실제 적용할 때 참고가 될 만한 유의점 및 지침을 논의했다.
This study aims to summarize potentials and cautions in applying neuroimaging methods to the field of counseling and psychotherapy. To do so, we systematically examined the research topics and methods from the 46 neuroimaging studies, published from January 1992 to June 2014, that investigated the effects of counseling and psychotherapy. The research topics were categorized into the years of publication, psychological symptoms and therapeutic interventions, and the research methods were categorized into sample size, intervention period, control/comparative group assignment, neuroimaging methods, experimental conditions, and the use of complementary measures. We found that obsessive-compulsive disorder (23.9%) and cognitive behavioral therapy (63%) were the most frequently studied symptom and intervention, respectively. The mean sample size, the most influential factor for statistical power, was 24.6, and the small number of subjects were often mentioned as a limiting factor. The average duration of intervention was 11.6 weeks, and 54.3% of the studies used the healthy control group. The most preferred neuroimaging method and experimental condition were functional magnetic resonance imaging (56.5%) and resting-state (43.5%), respectively. In almost all the studies examined (97.8%), treatment progress was measured to confirm the effects of counseling and psychotherapy. Based on these findings, we suggest best practices for designing rigorous neuroimaging experiments that aim to elucidate the neural mechanisms of counseling and psychotherapy, which will guide researchers to develop effective therapeutic interventions.
