뇌졸중 후 우울증은 물리치료 시 정신건강과 신체기능 향상에 부정적인 영향을 미친다. 그러나 현재까지 뇌졸중 환자의 우울증에 관한 국내 연구는 많지 않은 실정이다. 본 연구는 뇌졸중 후 우울증의 빈도를 파악하고 병명, 마비측, 유병기간, 인지 기능, 일상생활동작과의 연관성을 알아보기 위함이다. 대상자는 뇌출혈 및 뇌경색으로 판정받은 뇌졸중 환자로 연구목적에 동의하고 선정 기준에 적합한 35명을 대상으로 하였다. 실험 기간은 2010년 6월부터 2010년 9월까지 입원한 뇌졸중 환자를 대상으로 입원 일주일 내에 우울증 검사 및 기본 검사를 시행하였다. 측정 도구는 상지 기능검사, 일상생활동작검사, 인지검사, 벡 우울척도, 해밀톤 우울척도가 사용되었다. 성별, 병명, 마비측, 유병 기간에 따른 우울증 유무의 차이는 카이제곱 검정으로 하였으며, 인지 기능, 일상생활동작, 상지기능과 우울증의 연관성을 보기위해 Pearson 상관분석을 시행하였다. 연구 결과 벡 우울척도에서 25명(71.4%), 해밀톤 우울척도에서 20명(57.1%)의 뇌졸중 환자에게서 우울증이 보였다. 성별, 병명, 마비측, 유병 기간에 따른 우울증의 차이는 보이지 않았다. 그러나 우울증은 인지기능에서 높은 상관성을 보였으며, 일상생활동작과 상지기능에도 상관성이 있는 것으로 밝혀졌다. 또한 벡 우울척도와 해밀톤 우울척도 사이에 매우 높은 상관성이 있었다. 결과적으로 뇌졸중 환자에게 우울증상의 심각성을 알 수 있었고, 재활에 큰 장애요인임을 알 수 있었다.

Assessing poststroke depression has been complicated by cognitive deficits and several somatic stroke-related symptoms. The aim of this study is to investigate the relationship between symptoms of depression and mental, physical and daily function in life of stroke patients. Of hospitalized stroke patients between June 2010 and September 2010, 35 communicable patients were included in the study. Hamilton Depression Rating Scale(HDRS) and Beck Depression Inventory(BDI) were used to evaluate depression. Mini Mental Status Examination-Korea(MMSE-K) was used to evaluate mental function. Modified Barthel Index(MBI) and Manual Function Test(MFT) were used to evaluate physical function. 57.1% and 71.4% of the patients in HDRS and BID analysis showed to have depressive symptoms. There was no relationship between symptoms of depression and age, disease, paralytic side and onset time. There was a relationship between symptoms of depression and MMSE-K, MBI and MFT. The HDRS was very strongly correlate with BDI. As a result, depression is related to MMSE-K, MBI and MFT in poststroke patients. There is a pressing need for further research in clinical practice in this area of stroke care.
