본 연구의 목적은 뇌졸중으로 인한 편마비 환자를 대상으로 경사로, 계단에서의 PNF 하지 보행패턴을 적용한 보행훈련을 실시한 후 편마비환자의 보행능력이 어떻게 변화하는지 알아보고자 한다. 본 연구에서는 6개월 이상인 편마비 환자 30명을 대상으로 평지 보행군 10명(대조군), 경사로 보행군 10명, 계단보행 훈련군 10명을 무작위 배정하고, 각 실험군들은 기준의 신경계 운동치료 30분, PNF 하지보행패턴 30분을 실시하였고 주 3회, 4주 동안 실시하였다. 측정은 실험전과 후에 보행능력을 측정하기 위해 보장, 활보장, 입각기, 보장시간, 보행속도를 측정하여 자료를 분석하여 결과를 얻었다. 실험결과 보행능력이 계단보행군에서 실험전보다 실험후에 보장, 활보장, 보행속도에서 유의하게 증가하였고(p < .05) 입각기, 보장시간에서는 유의하게 증가하지 않았다(p > .05). 경사로보행군과 평지보행군에서는 모든 구간에서 유의하게 증가하지 않았다(p > .05). 세 그룹에 대한 실험후 변화량 검증에서는 모두 유의한 차이가 나타나지 않았다(p > .05). 결론적으로 세 그룹 중에서 계단 보행군에서 더 많은 보행의 개선을 보였다. 앞으로는 이런 훈련을 통해 다양한 환자에게서도 보행훈련이 이루어진다면 보행의 증진이 더욱 향상될 것으로 사료된다.

The purpose of this study was to evaluate the effects of a lamp gait exercise and stair gait exercise by proprioceptive neuromuscular facilitation(PNF) in gait ability of the patients with chronic stroke. Thirty patients with stroke were randomly allocated to three group: lamp gait exercise group(n =10) and stair gait exercise group(n =10) and control group(n =10). During four weeks, each group participated thirty minutes for third times per week. Subjects were assessed using pre-value and post-value measurement of step length, stride length, stance time, step time, velocity. These findings suggest that stair exercise group can be used to improve significantly step length, stride length, velocity of stair exercise group was significant difference (p < .05), but stance time, step time was not significant difference(p > .05). lamp gait exercise group, control group was not significant difference(p > .05). In comparison of three exercise group, there was not significant difference(p > .05). In conclude, stair gait exercise group by PNF improved gait ability, and thus it indicates that stair gait exercise group by PNF will be more improved through the other central neural injury patients.
