의자차 사용자의 50% 이상은 상지의 통증과 손상을 경험하였거나, 현재 문제를 가지고 있다. 이러한 의자차 사용자의 근골격계 손상을 예방할 수 있고, 역학적 효율이 높은 추진패턴을 선택할 수 있는 정보가 필요하다. 본 연구에서는 지체장애인의 주요 이동수단으로 사용되는 의자차의 추진 시 발생하는 상지관련 근골격계 질환을 예방하기 위하여, 추진주기의 시간적 변수를 이용한 최적의 의자차 추진패턴을 찾고자 실시하였다. 본 연구는 상지 근골격계/신경계 이상이 없으며, 심혈관계 질환 경력이 없는 20대성인 남자 24명을 대상으로, 속도에 따른 추진패턴 분석과 추진패턴에 따른 의자차 추진주기의 시간적 변수를 분석하여 최적의 추진패턴을 찾고자 실험하였다. 본 연구결과 여러 추진패턴 중 손상의 위험을 최소화시킬 수 있고 효율적인 최적의 패턴은 반원형이라고 할 수 있다. 추후 연구에서는 관절가동범위 변화, 에너지 소모율등 종합적인 실험을 통해 적절한 추진패턴을 찾는 연구가 필요하리라 사료된다.

The purpose of this study were to classify stroke patterns of manual wheelchair users and to determine if different patterns of propulsion lead to different biomechanics and cadence are different between stroke patterns. After having given written informed consent, 24 able-bodied male subjects participated in the study. Criteria for inclusion were: male sex, no previous experience in wheelchair propulsion, absence of any medical contraindications. To evaluate four different stroke patterns(semicircular, single-looping over propulsion, double-looping over propulsion, arcing), subjects performed four 1-min submaximal exercise blocks on a motor-driven wheelchair treadmill. The velocity was 0.67m/s or 1.33m/s for the lowand high-velocity group, respectively. Movement analysis was performed with a digital video camera, which was placed at the right side of the subjects, to record the hand movement in the sagittal plane. A stroke pattern that involves using a semicircular pattern with the hand below the pushrim during the recovery phase of the propulsion stroke was associated with lower cadence and more time spent in the push phase relative to the recovery phase. Therefore, it may be wise to train wheelchair users in this pattern of propulsion. Research is needed to determine the best technique for training wheelchair users in how to push using a specific pattern.
