치매 환자의 언어적 손상이 다른 신경언어장애에 비해 비교적 질병이 진행된 이후에 나타난다. 표현 언어보다는 잘 드러나지 않는 의미론적 측면의 손상 때문이라고 생각된다. 표준화된 언어 검사 과업을 통해서는 치매 노인의 이러한 언어 특징을 거의 잘 알아내지 못한다. 이러한 언어 특징은 담화 수준에서의 언어적 분석이 필요하다고 고려된다. 따라서 이 연구의 목적은 담화 분석법을 이용하여 치매의 유형에 따른 언어 특성을 살펴보고자 한다. 이 연구에서 선정한 치매 유형은 알츠하이머형 치매(AD), 뇌혈관성치매(VD)이며, 치매의 심한 정도는 치매임상평가척도(CDR: clinical dementia rating)에서의 척도 점수로 선정하였다. 연구 대상자는 AD집단 44명, VD집단 40명을 대상으로 실시하였다. 대상자들의 발화 수집 방법은 다음과 같다. 다양한 형식의 담화를 유도하기 위하여 일련의 활동에 따른 촉진 질문을 이용하여 발화를 수집하였다. 수집한 발화의 분석요소들은 담화의 긍정적 요소, 부정적 요소로 분석하였다. 연구 결과는 전체AD 집단과 VD 집단 간의 담화 특성을 비교한 결과, 담화의 긍정적 요소인 정보전달능력에서 집단 간 유의미한 차이가 나타났으나 주제유지능력에서는 유의미한 차이가 나타나지 않았다. 담화의 부정적요소에서는‘모른다’는 발화, 담화의 부정적요소 전체에서 집단 간 유의한 차이가 나타났다. 자발화를 통해 임상에서 치매인의 발화 능력에 대한 판단력을 향상시킬 수 있는 기초를 마련하였다고 본다.

Language damage of dementia is found later than other cognitive damages, which is because it is detected later than other neuro-speech disorders and moreover, because injuries of semantics is harder to be found than expressive speech. It is hard to find these speech specifications of dementia through standard speech tests. Therefore, they need considering by speech analysis on discourse. The purpose of this study is to examine closely into communication problems of dementia as a result of discourse specification on types and degrees of dementia through discourse analysis. The types of dementia in this study are Alzheimer’s dementia and cerebrovescular dementia, and the severe degrees are measured through the scores of‘Clinical Dementia Rating’. Research groups are AD group 44(10-14 per each CDR rating), VD group 40(10 per each CDR rating). The result of study follows below. First, after comparison of discourse specification on types of dementia between group AD and VD, we find out that there is no significant difference in subject-maintaining ability but in other hand, there is significant difference in informationdelivery ability. In negative elements of discourse, there is significant difference in‘I have no idea’speech and whole negative elements of discourse between group AD and VD. Group AD shows higher score in the average of‘I have no idea’speech than group VD. When it comes to the comparison of discourse specification between group AD and VD on degree, there is a significant difference in total speech quantity, information-conveying ability,‘I have no idea’speech. There is a significant difference between mild group AD and mild group VD for them while there is a significant difference between moderate groups for subject-maintaining ability. There is a significant difference between severe groups for‘I have no idea’speech.
