본 연구의 배경 및 목적은 다음과 같다. 휠체어와 착 석 시스템에 대한 정의를 보면 보행이 어려운 사람의 이동을 지원하고 적절한 자세의 앉기가 가능하도록 수 정하거나 조절해주는 보조기구로 되어 있다. 그러나 최 근 국내 휠체어 사용 장애인을 대상으로 연구한 결과 여전히 많은 수의 장애인이 욕창으로 인한 합병증이 발병하고 있으며, 휠체어 사용시 가장 불편한 신체 부위로 엉덩이와 허리를 선택하였다. 이러한 결과는 욕창 예방 방석을 사용하고 있으나 여전히 발병률이 감소되 지 않고 있으며, 앉기 자세에 대한 불편함이 일상생활을 방해하는 대표적인 요소임을 알 수 있었다. 따라서 본 연구에서는 휠체어를 사용하는 장애인을 위해 맞춤형 욕창 예방 방석의 디자인을 의학적 이론 기반 융합 연구를 통해 제안하고 이를 제작하였다. 디 자인에 대한 제안은 휠체어 사용 장애인의 특징과 욕 창 발생에 대한 의학적 배경, 앉기 자세에 대한 해부 학적인 요소를 근거로 디자인 하였다. 또한 욕창 예방 방석과 휠체어 프레임의 인체 공학적 특징을 기반으 로 한 이론적 근거를 바탕으로 욕창 예방 방석을 제 작하였다. 욕창 예방 방석 디자인의 기본적 개념은 휠 체어 사용 장애인의 엉덩이 좌면에 발생하는 압력을 감소시키고 적절하게 분배하며, 바른 앉기 자세가 가 능하도록 골반과 체간을 지지해주는 것이다. 이러한 이론적 근거를 바탕으로 맞춤형 욕창 예방 방석을 디자인하고 제작하였다. 비장애인을 대상으로 사용성 평가 실험 연구를 진행하였으며, 피실험자의 좌면을 스캔하여 방석을 제작하였다. 이렇게 제작된 맞춤형 방석과 현재 상용화되어 판매중인 조절형 욕창 예방 방석 2개 제품과 비교 테스트를 수행하였다. 실험을 수행한 결과, 궁둥뼈에 나타나는 최대압력지 수 값이 3개 제품중 가장 낮게 나타났으며, 압력을 분포시키는 분산지수도 가장 낮게 측정되었다. 그러나 엉덩이 좌면 전체에 나타나는 평균 압력은 다른 제품보다 다소 높게 나타나 추후 연구에서 이런 문 제점을 보완할 필요가 있었다. 향후 진행될 연구에서는 좌면의 전반적인 평균 압력 감소를 위한 디자인의 추가적인 설계가 요구되며, 이에 대한 테스트가 필요할 것으로 보인다.
The background and objectives of this research are as follows. The definition of a wheelchair and the seating system are a type of adaptive device that supports mobility of those who have difficulty ambulating and modifies or adjusts appropriate sitting. However, according to the results of a recent study on wheelchair users in Korea, a number of people with disability have complications from pressure ulcers, and pointed out hips and waist as the most uncomfortable parts of the body when using a wheelchair. These results present that although ulcer preventive cushions are used, incidence has not been on a decline, and the discomfort of the sitting posture is the main factor that disrupts every day life. The purpose of this research was to test the design of customized cushions for people with disabilities that prevents pressure ulcers. The proposals for the design was made considering the features of a wheelchair user, medical backgrounds for the occurrence of pressure ulcers, and the anatomical designs of the sitting posture. In addition, the pressure ulcer preventive cushion was built on a rationale based on the ergonomic features of the pressure ulcer preventive cushion and the wheelchair frame. The basic concept of the design of the pressure ulcer preventive cushion is to reduce and appropriately distribute the pressure caused from the eating surface and support pelvis and trunk for correct sitting posture. Customized pressure ulcer preventive cushions were designed and produced based on this rationale. Preliminary experiments were conducted to people without disabilities, and the cushions were produced by scanning the seat-contacted area of the subjects. A comparison test was performed on 1) tailor-made cushions and 2) commercially-shaped cushions that customize or adjust to prevent pressure ulcers. As a result of this experiment, the peak pressure index of ischial tuberosity values appeared the lowest of the three products, and the dispersion index also measured the lowest. However, since the average pressure index of the seat-contacted area presented higher values than other product, it was necessary to compensate for these problems in the further researches. Additional adjustments to the design to reduce the overall average of pressure on the seating-contacted area and the necessary test will be done in future researches. Measuring not only pressure, but also shear force and blood flow that affects the occurrence of pressure ulcer to increase the reliability and validity of the value data to the results is needed to verify the effectiveness of the pressure ulcer preventive cushion.
