영화 `아가씨`는 두 사람의 시점으로 바라보는 `히데코 아가씨의 탈출사건`의 전모이다. 이 탈출사건의 발단은 코우즈키의 강박증과 도착증에서 시작된다. 코우즈키는 조선인이 아니어야 한다는 불가능한 욕망으로 인해 강박증을 보인다. `일본인이 되어야 한다는 것` 또한 불가능한 욕망의 이면으로 이는 도착적 증상을 유발한다. 코우즈키의 도착증은 히데코를 대상화 시킨다. 이러한 일련의 과정이 유형화 되고 반복되는 것은 강박증적 증상이다. 코우즈키는 도착적 대상과 상황에 규칙성을 부여한다. 수집, 제책, 낭독과 재현을 통한 공개, 그리고 판매나 보유를 통해 가치를 획득하는 순차적 과정을 완성한다. 정신분석학적 관점에서 이 영화는 이모부의 억압에 얽매인 도착증자이며 히스테리 환자인 히데코가 자신의 욕망의 진실을 대면하고 욕망의 주체가 되어가는 과정이다.
