미국의 민주당과 공화당은 지난 100년 동안 전 국민의 의료보장을 두고 수많은 공방을 해왔었다. 최근 2017년에 취임한 미국 대통령 트럼프와 공화당은 『환자보호 및 적정의료법(Patient Protection and Affordable Care Act)』을 폐기 또는 수정하는 법안(American Health Care Act)을 하원에서 통과시켰다. 이 논문은 보편적 의료혜택에 대한 양당의 대결 양상을 통시적으로 고찰하고, 그 맥락에서 오바마 케어에 대한 트럼프와 공화당의 부정적 반응과 함께 트럼프 케어의 내용을 소개한다. 이 과정에서 오바마 케어의 핵심적인 내용인 국민의 의료보험 가입의무화와 메디케이드(Medicaid)의 확대가 완강한 보수주의자들의 반대로 폐지될 위기가 설명된다. 이후 결론으로서 미국 의료보험제도의 역사에 비추어 우리나라 국민건강보험법에 담긴 사회 공동체 의식의 소중함을 되새겨볼 것이다.

The American Democratic Party and Republican Party have engaged in a lot of political dogfights over the medical social security of the whole nation for the past 100 years. The current American President Trump taking office in 2017 and the Republican Party passed a bill (called the American Health Care Act), which is intended to repeal and replace the Patient Protection and Affordable Care Act, in the U.S. House of Representatives. This paper observes the aspects of confrontation between the parties about the universal benefits of medical services from a historical angle and in the same context introduces the contents of Trumpcare with Trump`s and the Republican Party`s negative reaction to Obamacare. Along the way, it is explained that the individual mandate and the Medicaid expansion, which play key roles in Obamacare, come to a crisis of being repealed owing to the opposition from the American diehard conservatives. After that, as a conclusion, I will meditate on the importance of the social solidarity in the National Health Insurance Act of South Korea in the light of the history of the American system of medical insurance.
