연구목적 : 본 연구는 거주환경에 따라 제주지역 소아청소년의 흡입 알레르겐 감작률의 변화 추세를 알아보고자 2010년과 2016년에 6년 간격으로 횡단면적 역학조사를 하였다.
연구방법 : 제주지역의 초, 중, 고등학교 13개교 재학생을 대상으로 6년 전과 동일한 연구 방법을 적용하였다. IRB 승인 후 학생 및 보호자가 모두 서면으로 동의한 경우에 한해 흡입 알레르겐 18종에 대해 피부단자시험(Skin prick test)을 하였다. 피부단자시험에 유효한 최종 분석 대상자는 2010년에는 1,549명, 2016년에는 1,087명이었다.
연구결과 : 흡입 알레르겐 감작률은 43.7%(2010년)에서 51.8%(2016년)로 크게 증가하였다. 2010년도에 비해 2016년도에 유의하게 높은 감작률을 보인 알레르겐은 D. pteronyssinus(33.8%/39.6%), D. farinae(25.0%/31.6%), 일본삼나무(15.8%/20.5%), 큰조아제비(3.1%/8.2%), 우산잔디(1.4%/3.3%), 오리나무(0.5%/2.6%), 참나무(1.1%/2.1%)이었다. 거주환경에 따른 감작률은 감귤지역이 50.2%(2010년)에서 55.4%(2016년)로 가장 높았으며, 도심지역은 40.9%(2010년)에서 49.4%(2016년), 해안지역은 36.3%(2010년)에서 46.9%(2016년)로 유의하게 증가하였다. 일본삼나무의 감작률은 감귤지역이 타 지역에 비해2.61배(2010년), 2.97배(2016년)의 높은 위험도를 보였다. Alternaria의 감작률은 해안지역이 타 지역에 비해 3.45배(2010년), 2.12배(2016년) 높은 위험도를 보였다. 집먼지진드기류는 2010년도에는 감귤지역이 1.52배 높은 위험도를 보였으나 2016년 도에는 지역별 차이가 없었다.
결론 : 동일 지역 내에서도 거주환경에 따라 흡입 알레르겐 감작률은 차이가 있었으며, 6년 전보다 감작률이 크게 증가한 것을 확인하였다. 향후 거주환경에 따른 맞춤형 알레르기질환 예방관리 프로그램이 필요하다.
Objective : This study is to discover the trends and changes related to aeroallergen as they relate to the residential environment of school children in Jeju Province, using a cross-sectional study with a span of 6 years, from 2010 to 2016.
Methods : The same study method employed 6 years ago was applied to the students from 13 elementary, middle and high schools in the Jeju Province. The skin prick test was performed for 18 types of aeroallergen under the approval of Institutional Review Board (IRB) and the consent of the subjects and their parents. The subjects for the effective final analysis were 1,549 in 2010 and 1,087 in 2016.
Results : The results indicate that the sensitization rates of aeroallergen were on a sharp increase: from 43.7% in 2010 to 51.8% in 2016. As for the sensitization rate, the risk degree for Japanese cedars showed 2.97 times in 2016, compared to 2.61 times in 2010. That for Alternaria showed 2.12 times in 2016 to 3.45 times in 2010. The allergens of the high sensitization rate were listed in the order of D. pteronyssinus, D. farinae, Japanese cedar, Timothy grass, and Alternaria.
Conclusions : The residential environment makes meaningful significances in the sensitization rates of aeroallergen, showing high increase in sensitization rates. This suggests that allergic disease prevention and care programs should be implemented in order to cope with each residential environment.
