이 연구에서는 유아의 비순응 문제를 다루기 위해 관련된 근거기반 치료 프로그램을 고찰하여 이를 토대로 국내 상황에 맞게 부모 상담 프로그램을 개발한 후 그 개입 효과를 검증하고자 하였다. 프로그램 개발을 위하여 유아동의 비순응, 적대적 반항 및 품행 문제에 대한 근거기반 프로그램을 고찰 하였다. 프로그램은 두 부분으로 구성되었는데 하나는 프로그램 참여자가 자녀의 비순응 문제를 다루기 위해 익혀야 할 기술을 매뉴얼을 통해 동일하게 익히는 것이고(‘공통 모듈’, 6회기), 다른 부분은 참여자가 개별 회기를 통해 각각의 상황에 맞게 맞춤식으로 익히는 것(‘고유 모듈’, 2회기)이다. 프로그램 효과 검증을 위하여 어린이집에 재원중인 만2~3세, 외현화 문제행동의 고위험 유아 10명의 어머니가 연구에 참여하였다. Wilcoxon Signed Rank 비모수 검정을 통해 사전-사후 점수를 비교하여 얻은 주요한 결과는 다음과 같다. 첫째, K-CBCL 1.5-5로 평가한 공격성 문제, 외현화 문제, 및 DSM 반항문제에서 통계적으로 유의미한 감소가 나타났다. 둘째 공격성 문제와 DSM 반항문제에서의 효과 크기가 중등도로 나타나 임상적 유의도가 확보되었다. 셋째, 프로그램 참여 어머니를 대상으로 평가한 프로그램 만족도에서 긍정적인 평가가 산출되었고, 특히 개별 상황에 맞춰 개인상담으로 진행된 것에 대한 만족도가 큰 것으로 보고되었다. 연구의 제한점과 향후 연구를 위한 제안이 논의된 가운데 상담자가 진행한 근거기반 개입의 효과가 입증된 만큼 이 프로그램의 보급과 시행이 비순응 유아의 문제를 다루기 위한 효과적인 치료로 제안되었다.
The purpose of this study is to develop an evidence-supported parent intervention program using cognitive behavioral therapy for non-compliant preschoolers and to verify its effect. To develop this program, evidence-supported programs for non-compliance and oppositional defiant and conduct problems were reviewed. This program has two components; in the first part, every participant trained for skills using procedures from a manual(‘common module’ 6 sessions). In the second part every participant trained for skills using tailored contents for each participant(‘customized module’ 2 sessions). The participants of this study were 10 preschoolers' mothers whose children aged 2-3 years are at high risk for noncompliant behaviors. The effect of this intervention was verified through a pre and post-test using Wilcoxon Signed Rank. The significant findings of this study are as follows. First, the preschoolers showed significantly lower scores in aggressive problem behavior, externalizing problem behavior, and DSM defiant behavioral problem. Second, the effect size of aggression problem and DSM defiant behavioral problem was moderate and clinical significance was earned. Third, program satisfaction proved to be positive especially for the tailored personal sessions. Limitations and suggestions for future study were mentioned. Effects of this program for the non-compliant preschoolers were identified, so the dissemination and implementation of this program are suggested for non-compliant preschoolers.
