임상간호사의 수면의 질에 영향을 미치는 요인

Purpose: The purpose of this study was to identify sleep quality, job stress, symptoms of stress (SOS), and coping and to examine factors influencing sleep quality in clinical nurses. Methods: A cross-sectional study was conducted with 265 clinical nurses at three general hospitals. A self-administrated questionnaire was used for data collection, and data were analyzed using descriptive statistics. Pearson correlation coefficients, t-test, ANOVA, and backward multiple regression with the SAS statistical program. Results: Sleep quality showed significant differences according to the sociodemographic characteristics of age, length of work, education, current workplace, current job position, shift work, and marital status. Sleep quality was negatively associated with SOS and job stress. Significant factors influencing sleep quality of the clinical nurses were cognitive-behavioral SOS, physiological SOS, inappropriate rewards, physical work environment, and shift work. Conclusion: The findings indicate that cognitive-behavioral SOS, physiological SOS, job stress regarding inappropriate rewards and physical work environment, and shift work are useful to predict levels of sleep quality of clinical nurses. These factors should be considered when developing nursing interventions to improve sleep quality of clinical nurses through stress management.

Purpose: This study was conducted to test the effects of a community-based depression prevention program on community people and the effectiveness was measured by assessing depression, self-esteem, and quality of life. The theoretical framework supporting the program was Ellis` REBT (Rational Emotive Behavior Therapy) theory (1962). Methods: A nonequivalent control group pretest-posttest design was used. Study participants were 29 community residents (experimental: 14, control: 15). The depression prevention program consisted of music activity, bibliotherapy, and cognitive behavior therapy and was provided for 6 weeks (a twelve-session). Data were collected before and after the program and 3 months later for follow-up testing. Data were analyzed using the SPSSI/WIN 12.0. Results: There were no differences in outcome variables between the experimental and control groups. From the additional analyses, statistically significant differences by time point were observed for Self-esteem and Quality of life at 3 months later for experimental group. And 50% of the participants with depression symptoms no longer scored in the depression symptoms in the experimental group. Conclusion: The study findings indicate that depression prevention programs are positive in reducing depression in people suffering from depression symptoms. However some modifications are required to increase effectiveness.

Purpose: This study was done to examine the effects of a literary therapy intervention on ego-resilience and maladaptive behavior among children of broken homes. Methods: The intervention consisted of 16 sessions designed to achieve a positive self-concept and was constructed as an interaction between participant and certain literature including poetry, narratives, dramas, art therapy, and willingness to share it with others. The study was carried out from May 2 to July 25, 2006. The participants were 36 children resided in a vulnerable region in G city. The collected data for Ego-resilience, Korean-Children Behavior Checklist (K-CBCL), Korean-Youth Self-Report (K-YSR) were respectively analyzed using descriptive statistics, t-test, repeated measures ANOVA with Post Hoc, and ANCOVA with the SPSS/WIN version 12.0 program. Results: After the intervention, there was a significant difference between the experimental and the control group for Ego-resilience, K-CBCL, and K-YSR. There were significant differences according to time process in ego-resilience and K-YSR. Conclusion: The results indicate that literary therapy is an effective nursing intervention for enhancing ego-resilience and reducing maladaptive behavior in children of broken homes.

Purpose: The study was done to identify factors influencing the quality of life of people with mental disabilities who use mental health centers. Methods: Data were collected by questionnaires from 288 people with mental disabilities in Busan and Kyungnam Provience. Data were analyzed using t-test, ANOVA, Pearson correlation coefficients, multiple linear regression with the SPSS/WIN 14.0 program. Results: The mean score for quality of life was 11.3±1.70, and the mean empowerment score was 59.7±8.44. The significant factors influencing quality of life were gender, age, residential status, number of support people, side-effects of drugs, service type, and empowerment. These factors explained 43.9% of the variance. Conclusion: The results suggest that in development and practice of program to increase the quality of life of people with mental disabilities a variety of factors should be considered.

Purpose: The purpose of this study was to identify experiences of immigrant women from Philippines in childrearing. Methods: The study design was a qualitative research. Participants were 15 Filipino immigrant women living in rural Korea. Average age was 37 and average duration of marriage was 11 years. Data were collected through in-depth interviews and participants were interviewed once or twice between December 2009 and February 2010. Data were analyzed with Colaizzi`s (1978) phenomenological methodology. Results: The Four themes-clusters were identified from fourteen themes. The theme-clusters (themes) were 1) Responsibility of a mother as a foreigner (being a role-model, patience, strict upbringing); 2) Practical problems of child-rearing (parenting alone, language difficulties, financial difficulties, lack of husbands` support, cultural obstacles; 3) Worries about their children (children`s emotional problems, speech disorders, maladjustment at school); 4) Hopes for the future (self-satisfaction, expectations for their children, social support). Conclusion: The results help in understanding the situation of immigrant women from the Philippines when they nurture their children and suggest the necessity of programs focusing on improving mother-child relationships, especially as related to mothers` limited language skills. Further comprehensive education programs to improve better father-child relationships in relating to each other should be developed.

Purpose: The purpose of this study was to identify the relationship between hope, self-care agency, and mental health in patients with chronic schizophrenia. Methods: The participants were 118 patients with chronic schizophrenia receiving care at a community mental health center. Data were collected from March 5th to May 29th, 2009 and were analyzed with using descriptive statistics, Pearson`s correlation coefficients, and stepwise multiple regression. Results: A significant positive correlation was found between hope and self-care agency; and a significant negative correlation between hope and mental health. Hope significantly predicted by mental health(p<.001) explained 34.9% of the variance in mental health. Conclusion: As the results indicate hope is significantly correlated with self-care agency and mental health in patients with chronic schizophrenia. It is recommended that intervention programs should be developed to increase hope and to evaluate more precisely factors related to mental health in this population.

Purpose: The purpose of this study was to investigate the current status of international marriage migrant women living in Daegu and Kyungpook area, and to identify the impacts of social support and life satisfaction on their depression in this population. Methods: Research objectives were accomplished by conducting an anonymous questionnaire survey with 173 international marriage migrant women. Data were analyzed using descriptive analysis, Pearson correlation coefficients and stepwise multiple regression with SPSS/WIN 18.0. Results: Compared to international marriage migrant women living in other areas, international marriage migrant women living in Daegu and Kyungpook area had differences in their host country, level of education and spouses` occupation. The most frequently used social support programs for participants were classes in Korean language (Hangul), Korean culture and tradition and on the National Health Insurance Service. Women who received higher levels of social support reported less depression and greater life satisfaction compared to women receiving lower levels of social support. The significant predictors of depression were social support and life satisfaction showing 27.8% of the variance in depression. Conclusion: Based on the research finding, the author suggests the necessity of developing social service program based on the women`s characteristics and mental health needs.

Purpose: The purpose of this study was to identify the relationship of self differentiation, self esteem and mental health in adolescents. Methods: Data were collected from 829 adolescents and analyzed using descriptive statistics, t-test, ANOVA, Scheffe`s test, Pearson correlation coefficients and multiple regression with the SPSS 14.0 program. Results: Mental health differed according to grade, level of father`s and mother`s education. Self differentiation and self esteem had a significant negative correlation with mental health. Stepwise multiple regression analysis showed emotional cutoff, family projection, cognitive emotional function, and family regression in self differentiation, self esteem, level of mother`s education explained 24.4% of the total variance in mental health. Conclusion: The findings indicate that self differentiation and self esteem have an influence on mental health. We need to develop and provide nursing intervention programs to prevent and manage adolescent`s mental health problems and include the family in these programs.

섭식장애 환자의 알코올 문제 관련요인 분석

Purpose: The purpose of the study was to investigate the psychological factors such as eating psychopathology, depression, obsessive-compulsion, sleep problem, and stressors that affect the alcohol problems in patients with eating disorders. Methods: Patients with eating disorders (n=347) who visited "M" clinic for eating disorder were participated in the study. Data were collected from February 2008 to May 2010 using Eating Disorder Inventory-2, Beck Depression Inventory, Maudsley Obsessional-Compulsive Inventory, and Alcohol Use Disorder Identification Test-Korean. Results: Of the participants, 17.3% had an alcohol problem, 9.2% of them were diagnosed with alcohol dependency. Age, history of mental disorder in family, stress for love relationship, difficulties falling asleep, and ``impulse regulation`` (one of eating psychopathologies) were significant associated factors of alcohol problem, but depression and obsessive-compulsion were not significant. Conclusion: Future interventions for patients with eating disorders should focus on assessing alcohol problems. Assessment should be non-judgmental, especially for those patients with high levels of impulse dysregulation, difficulties falling asleep, and stress in love relationship in order to prevent their early drop-out for the treatment. Alternative treatment for sleep problem and stress management also may be helpful for the patients with alcohol problem and eating disorders.
