보건통계학 교육시 R의 활용에 대한 연구(1)

We can use R package as a statistical package on the health statistical education for college students. R is an interactive mode package and graphical presentation tools are powerful in R. The greatest advantage is that R is a general public license package. We need to consider R package as a standard statistical package on the health statistical education for college students.

Objective: This study was tried to survey nurse`s satisfaction on the ENR(electronic nursing record) system in general hospital in Daegu. Methods: 250 nurses who were working in the A general hospital in Daegu participated to survey through Questionnaire for User Interaction Satisfaction about satisfaction of electronic nursing record system. Results: Mean score of Questionnaire for User Interaction Satisfaction was 3.18 and this result was statistically higher than middle score of 3. Score of terminology & message was the highest and score of capabilities was lowest in all categories of Questionnaire for User Interaction Satisfaction. Results of stepwise multiple regression between overall evaluation and other categories were that categories of learning, capabilities, screen influenced category of overall evaluation and category of learning was the biggest influence variable. Conclusion: Users seem to be satisfied with the electronic nursing record system and this results could help to modify better electronic nursing record system in A hospital.

The diplotype of UGT1A1 and UGT1A9 genes may predict an increased risk for Lung cancer

Uridine diphosphoglucuronosyltransferases (UGTs) are a superfamily of enzymes that catalyze the glucuronidation of xenobiotics and endobiotics. To reveal occurrence of UGT1A polymorphisms, we retrospectively aimed to characterize UGT1A1-UGT1A9 haplotype structures in Koreans and gain insight on their association with increased cancer incidence. We genotyped UGT1A1 (-3279T>G, -3156G>A, -53(TA)(6>7), 211G>A and 686C>A), and UGT1A9 (-2152C>T, -275T>A and -118T(9>10)) variants by direct sequencing in 98 lung cancer patients and 154 healthy subjects. The haplotype structure across the UGT1A locus was constituted using 4 haplotype-tagging SNPs (htSNPs). For polymorphisms in UGT1A1 and UGT1A9, no significant differences in allele frequency were found between the two groups. The strongest linkage disequilibrium (LD) was discovered between UGT1A1 -53(TA)(6>7) and 211G>A, and lowest LD was observed between the UGT1A1 -53(TA)(6>7) and UGT1A9-118T(9>10) variant. Eight UGT1A1-UGT1A9 haplotypes were provided and the three most common haplotypes were D-T-6-G, N-T-6-A, and N-G-6-G (73% of all haplotypes) in two groups. Ultimately, no significant differences exist in haplotype frequency between cases and controls (likelihood ratio statistic 20.06, df=15, p=0.08). Logistic regression analysis presented a statistically significant association between UGT1A1-UGT1A9 diplotypes including 4 minor alleles and increased lung cancer incidence (OR 31.32, 95% CI 4.34-226.23). There was statistically significant difference in the distribution of the UGT1A1 and UGT1A9 diplotype frequency between cases and controls. These results suggest that the diplotype in the UGT1A1 and UGT1A9 gene might be associated with an increased risk for lung cancer among Korean.

Objective: This paper aims to compare the performance of regression-based statistical approaches that were currently used or advocated to adjust a treatment effect. Methods: The six methods used to compare their relative performance were: excluding treated individuals from data, no adjustment for treatment effect, modelling treatment as a covariate(indicator variable), non-parametric adjustment of treatment, adding a constant value to measurements for treated individuals, and censored normal regression. We applied these methods to real genetic and clinical data from Yonsei cardiovascular genome center to demonstrate a pattern of their behaviour. Results: Two of the adjustment methods were more powerful than other methods for analysis of genetic association with serum lipid profiles. These were: no adjustment to the observed lipid profiles in treated subjects, non-parametric adjustment method based on averaging ordered residuals. Conclusion: Non-parametric adjustment method based on averaging ordered residuals and no adjustment to the observed lipid profiles in treated subjects can effectively adjust the distorting effect of lipid-lowering drug and recover a marked loss in statistical power. Also, in genetic association studies of continuous traits that distortion arising from a treatment effect really matters, we proposed to use the appropriate methods that are more effective and straightforward to implement.

Objective: This study was carried out to measure job satisfaction and quality of life using WHOQOL-BREF for quality of life. Methods: The subjects for this study were 84 people who worked in the Ulsan and Kyungnam area. The WHOQOL-BREF is a short version of the WHOQOL. It consists of 26 items, each with 5-point Likert responses. The instrument assesses four broad domains, namely physical health(7 items), psychological(6 items), social relationship(3 items) and environment(8 items). Results: The results for this study were as follows: the factors that influenced the quality of life, were especially age, marital status, monthly earning, years in current position, night shifts and intention of quit job. There are significant differences between quality of life and job satisfaction. Conclusion: In conclusion, dental hygienists should be improve job satisfaction and quality of life.

유전자료를 이용한 악성 간암환자 나무구조 생존예측 모형연구

A tree-structured method for analyzing censored survival data is attractive for several reasons. The association between survival time and covariates is easy to interpret even if the covariates are numerous and there are interactions among them. Moreover a tree-structured method using gene expression data gives additional insight into the role of the covariates. Recent SNP data analysis makes it possible to find out large numbers of gene related with human cancers and to support accurate diagnosis of human cancers according to their several pathological judgements. The purpose of this study was designed to evaluate the prognosis of HCC in relation to treatment methods and their affecting gene and clinical factors by tree structured model. We could identify a feature set of 44 genes to distinguish the status of HCC and non-tumor liver tissues. Our proposed survival tree model shows the genes related with clinical, pathological data and risk factors of HCC. These findings could be available to predict prognosis of HCC and give valuable information to justify the treatment strategy for clinician.
