국내 광주, 전남 지역에서 비정상적인 자궁경부 표본과 연관된 인 유두종 바이러스의 유전자형 분석

Objectives: This study investigated the association between genotypes of human papilloma virus (HPV) infection and development of cervical abnormalities in patients in Gwangju and Jeonnam, Korea. Methods: Cross-sectional study was performed using HPV DNA chip test (7,818 samples) and histological analysis of obstetrics patients in Gwangju and Jeonnam, Korea. Single and multiple HPV infections were analyzed in patients with cervical intraepithelial neoplasia and cervical cancer. Results: Prevalence of HPV peaked in women 10s-20 years of age, decreased in women 30 years of age, and increased in women 60 years of age in Gwangju and Jeonnam, Korea. One of the most interesting results of the histological diagnosis showed that genotype distribution in the high-risk group increased with high-risk lesions. These results do not mean that multiple infections are more dangerous but may be an important factor in specific high-risk HPV genotypes. Conclusions: The prevalent HPV genotypes were HPV 16, 58, 18, 33, and 52 in the histological analysis.

부산지역 대학생의 도박행동유형과 건강위험행동이 도박문제에 미치는 영향

Objectives: This study aims to examine college students` gambling behavior and investigates the effect of factors on problem gambling, such as personal characteristics, gambling history, and health risk behaviors. Methods: The subjects of the study were students selected from six colleges in Busan, Korea. A total of 431 questionnaires were used in the final analysis. Results: First, the results showed that more than 60% of respondents had gambled in the past year. The most popular forms of gambling were the lottery, followed by card betting, sports betting, and online gambling, betting on horse-racing, cycle-racing, and motorboat-racing games, and casino games. Second, frequent smoking and drinking increased the incidence of the problem gambling. Third, it was revealed that gender and age were negatively correlated with gambling, while the frequency of gambling activities and the number of previous gambling experiences were positively correlated with problem gambling behavior. Conclusions: Ongoing preventive screening and early intervention must be made a priority to prevent college students` gambling behavior from reaching serious levels.

치과 진료실 감염관리 지표에 대한 실천정도의 연구

Objectives: This study aims to find out the general characteristics of dental hygienists related to practice of infection control in dental office. Methods: A questionnaire survey was performed to the dental hygienists who participated in the refresher training conference held by the Daejeon City Commission in October 2012. A total of 171 questionnaires were collected and the data were analyzed using PASW 18.0 Cross-tabulation analysis (χ2) which significance level was 0.05 was applied to the data in order to verify the statisict al significance of the survey method. Results: In terms of compliance with general routine requirements for infection prevention in dental office, the proportion of respondents who chose the answer ‘Use protective goggles and apron for apparatus washing’ was the largest (77.7%), while the proportion of those who chose the answer ‘always pretreat apparatus with disinfection solutions’ was equivalent to 49.1%. Regarding disinfection to the surface of dental unit chair and waterline, the respondents who chose the answer ‘I sometimes perform surface disinfection’ accounted for the largest proportion, 64.3%, and those who chose the answer ‘I do not cover surfaces’ accounted for 43.3%. Conclusions: Based upon the findings summarized above, it is concluded that preventive and proactive actions made and taken by dental hygienists for the sake of dental infection control contribute greatly to preventing nosocomial cross infections and providing safer hospital settings for both patients and dental care professionals.

한국인 성인 남녀의 흡연관련 사망에 관한 연구

Objectives: Cigarette smoking has been widely recognized as a major risk factor for lung cancer and other diseases in Western countries. In Korea, male cigarette smoking prevalence was among the world`s highest between 1980 and 1990 and smoking has also become a strong risk factor for lung cancer and cardiovascular diseases. The objective of the study was to calculate the smoking-attributable mortality in 2012 in Korea. Methods: Number of smoking-attributable deaths were calculated by applying the percentages of population attributable risks (PARs) to the estimated number of deaths by diseases in 2012. In this study, PARs were obtained by using relative risks from the Korean Cancer Prevention Study and the Metabolic Syndrome Mortality Study, and population smoking prevalence surveyed in 1985 conducted by Korean Institute of Tuberculosis. Results: The smoking-attributable mortality was 58,155 death in 2012. Among adult male, the smokingattributable mortality (49,704) represents 34.7% of total 2012 mortality, whereas the smoking-attributable mortality (8,451) for adult females was 7.2%. Smoking was supposed to be responsible for 41.1% of all male cancer and 33.4% of all male cardiovascular diseases, whereas smoking for 5.1% of all female cancer and 5.4% of all female cardiovascular diseases in Korea. Conclusions: Smoking actually represents a remarkable burden of avoidable deaths in Korea. Smokingattributable mortality appears to continue increasing by the next 10 to 20 years.

퇴원환자 요약정보를 이용한 이탈환자 특성과 관련 요인 연구: 일개 대학병원 사례

Objectives: Patients who are discharged from hospital prematurely have become strategically important for hospital customer relationship management. The purpose of this study was to investigate the characteristics of patients who were discharged prematurely and the associated factors using patient discharge data. Methods: This study used a retrospective, cross-sectional design. Primary data source comes from medical records of patients who were discharged from a university medical center during 2011. Data were analyzed using chi-square test, T-test, Mann-Whitney U test, and multivariate logistic regression to find significant patient characteristics and factors associated with premature discharge. Results: Final research subjects were 18,104 patients after excluding duplicated discharge patients, death patients, and discharges against medical advice. Male sex, older age, and living further away from Seoul were positively associated with premature discharge. Patients who were admitted via emergency room, due to car accident, and referred by other medical providers were more likely to discharge prematurely. Those who were diagnosed with neoplasm or circulatory system illness, and having comorbidities or complications were related to premature discharge. Other factors, such as having consultation treatment, non-favorable treatment outcome, and staying in intensive care unit had higher odds of premature discharge. Conclusions: It is suggested that hospitals monitor patients who would be more likely to be discharged prematurely, and need to address any quality issues during hospitalization if they lead patients to discharge early.

성인 여성의 피부 건강상태에 영향을 주는 요인

Objectives: The aim of this study was to examine whether there is an association between lifestyle and skincare behaviors and skin health status of adult women. Methods: A sample of 102 adult women aged 20s to 50s years old were conveniently recruited at several skin care center and cosmetics shops in Daegu, Korea. The subjects were examined for skin health status by Aramo-SG (AramHuvis Inc., Korea). The demographic and lifestyle characteristics and skincare behaviors surveyed by self-administered questionnaire. Univariate and multivariate analyses were performed to determine the factors related with skin health status using SPSS 18.0. Results: In univariate analysis, Aged over 30s (p<0.001), married (p<0.001) and appropriate drink (p=0.040) were positively associated with skin health status among demographic and lifestyle characteristics. Among the skincare behaviors, experience of homecare only significantly associated with skin health status (p=0.007). In multivatiate analysis, age and experience of homecare were independently significantly associated with skin health status after adjusting for other variables. Conclusions: An appropriate drink and experience of homecare were positively associated with management of skin health. Therefore, it is important to have proper skincare behaviors to keep sound skin condition.

자연사 및 외인사의 계절변동에 관한 연구

Objectives: The purpose of this study was to examine the seasonal variation between natural cause of death, external cause of death, and intentional self-harm and month and season. Methods: We evaluated the monthly and seasonal changes on natural cause of death, external cause of death, and intentional self-harm in Korea. The analysis was based on data of 258,994 deaths from 2011 vital statistics of Korea. Results: Number of average death in one day was 623 in natural causes of death (NCD), 46 in external causes of death (ECD) and 46 in intentional self-harm death (SHD). The death rate per 100,000 was 446.3 in NCD, 32.7 in ECD and 31.4 in SHD. There was a significant difference between gender, month & season and distribution of internal & external cause of death (p<0.01). Gender ratio (male/female) of death rate was 1.16 in NCD and 2.20 in ECD and 2.15 in SHD, and coefficient of variation of death was 7.5, 16.0 and 23.1, respectively. In the incidence of cause of death by season, NCD was highest in the winter (26.7%), ECD and SHD was highest in the summer (26.1%, and 28.9%, respectively). Conclusions: Adjusting to the varied lengths of each month, we used mean number of death per day. There was a winter peak (673.4 deaths) and summer trough (578.8 deaths) for NCD, and summer peak (48.3) and spring trough (43.9) for ECD, and summer peak (51.5) and winter trough for SHD (35.8).

미취업 정신장애인의 건강상태, 경제수준, 직업능력이 취업의사에 미치는 영향

Objectives: The purpose of this study was to explore influencing factors associated with willingness to work targeting unemployed people with mental disabilities in Korea. Methods: Data for this study were obtained from the 2011 panel survey of employment for the disabled. Mentally disabled people only who had no job (n=78) were included in this study. Data were analyzed by descriptive statistics, χ2-test, and hierarchical multiple logistic regression. For the analyses, SPSS ver. 20 was used. Results: The main findings were as follows. Men in their 40s and high school graduates were dominant in this study. Overall, their socio-economic status was very low. The majority responded that their interpersonal/ organizational skills were far below average. In the hierarchical logistic regression models, subjective health condition was the only variable which was statistically significant to willingness to work. Conclusions: Based on these findings, it is important to provide government-led community-based health promotion and vocational rehabilitation programs for unemployed people with mental disabilities. In addition, considering the characteristics and nature of mental disabilities, psycho-socially supportive as well as physically friendly environments should be developed.
