치매환자 특성과 재원일수 결정요인의 병원 종별 비교

Objectives: The purpose of this study is to investigate the differences in characteristics of dementia patients and the factors that affect length of stay among three types of hospitals: general, district, and long term care hospitals. Methods: From National Patient Survey database prepared by Korean Institute for Health and Social Affair, discharge summary data of 7,279 dementia patients are extracted and analyzed. Three logistic regression models with binary dependent variables indicating different types of hospital were used to analyze the differences in characteristics of dementia patients among hospitals types. Three linear regression models with length of stay (LOS) as dependant variable were used to determine the factors that affect LOS of the three types of hospitals. Results: The long term care hospitals had the highest level of LOS, mortality rate and disease severity. The severity had a positive effect to LOS in general hospitals and in long term care hospitals. The Medical Aid Program affected positively to LOS in acute care hospitals, but had no significant effects on the long term care hospitals. Conclusions: A mismatch between severity level of disease and hospital types exists in the care of dementia patients. Health policies intending to encourage for dementia patients to use appropriate level of hospitals services have to be established and implemented.

Objectives: Health expectancy, an intuitive and meaningful summary measure combining the length and quality of life, has become a standard in the world for measuring population health. The purpose of this study is to review on the disability free life expectancy (DFLE). Methods: This article critically reviews the literature and practices around the world for measuring and improving DFLE and synthesizes that information as a basis for recommendations for the adoption and adaptation DFLE. The DFLE estimates in this article were derived from the 2014 Eurostat Statistics Database and 2014 world health statistics of World Health Organization. Results: Korean and British have a same life expectancy 81 years in 2012. But Korean have a longer healthy life expectancy than British: 73 and 71 years, respectively. If we consider life without disability, Korean live 90.1% of their years without disability, British only 87.8%. At a European level similar data can be found: on average women live longer but have a worse longer time of life with disability. Conclusions: Different classifications of healthy and unhealthy states could result in substantial differences in healthy life expectancy. Some caution should be exercised in defining health status.

Objectives: The purpose of this study was to investigate the levels of clinical stress, fatigue, satisfaction and self-efficacy of nursing students in clinical practice, and to examine the relationship of the variables. Methods: The participants were 128 3rd-year female nursing students. Data were collected from December 5 to December 20, 2013 by taking a survey with the self-administered questionnaires. Data were analyzed with SPSS WIN 18.0 program. Results: The results of this study were as follows: The mean score of clinical stress was 2.74(±0.62), fatigue was 3.30(±0.52), satisfaction in clinical practice was 3.06(±0.48), and self-efficacy was 3.29(±0.52). The participants having satisfaction with nursing as a major highly were shown higher satisfaction in clinical practice and self-efficacy. There was a negative correlation between clinical stress and satisfaction in clinical practice, between fatigue and self-efficacy, and a positive correlation between satisfaction in clinical practice and self-efficacy. Conclusions: The results of this study indicate that the level of clinical stress, fatigue, satisfaction and self efficacy in clinical practice can be influenced on the quality of nursing practice. Thus there is a need to develop program to reduce the clinical stress and fatigue and to improve satisfaction and self-efficacy in clinical practice.

Objectives: The purpose of this report is to monitor the status of blood management based on blood supply, usage, and wastage data provided by Korea Blood Inventory Monitoring System (KBIMs). Methods: KBIMs is the ongoing national surveillance system for blood management. The current report includes data from 25 sentinel hospitals during 2009-2012 years. In this sample, the data collected in each hospital were summed and defined on the weekly basis for the use of the control chart. Results: Since the amount of blood usage covers over 99 percent of the supply, they appear to have similar control chart patterns. Blood type A, B and O show obvious shifts in mean supply and usage which occur beyond the center line in 2012. Blood wastage is out of control in 2010 and 2012. Interestingly, this does not occur in the winter season and the pattern of blood wastage has significant seasonal difference (p=0.001). Conclusions: Findings suggest that the blood usage has an increasing tendency in all blood types except AB in 2012, while blood wastage has seasonal variations. Even though such state may not stay the same, this analysis provides enough warrants for further study.

일부 대학생의 스마트폰 중독이 수면시간과 수면부족에 미치는 영향

Objectives: This study was performed to identify the effects of smartphone addiction on sleeping time and sleep deprivation among some college students. Methods: The study subjects were 287 students in a college who were using the smartphone, and data was collected by self-administered questionnaire. The used statistical analysis were t-test, ANOVA, Pearson``s correlation analysis and multiple regression analysis to find the association of smartphone addiction with sleeping time and sleep deprivation. Results: The smartphone addiction scores of study subjects were 32.9±6, and 17.1% of subjects was at-risk group and 5.6% was high-risk group in classifying risk groups. Mean hours of sleeping time were 6.4±1.2 and mean scores of sleep deprivation scale were 8.4±3.1. The smartphone addiction had negative correlation with sleeping time, but it was not statistically significant. However, it showed statistically significant positive correlation with sleep deprivation scale. As results of multiple regression analysis, the smartphone addiction was statistically significantly associated with sleeping time (β=-0.033, p=0.001) and sleep deprivation scale (β=0.127, p<0.001). Conclusions: This study showed the smartphone addiction in college students was negatively associated with sleep. It is necessary to perform the further researches and to develop the programs for solving the problems due to smartphone addiction.

CDSS 실용화를 위한 ArdenML XSLT 개발 -VB.NET을 활용한-

Objectives: The ArdenML is an HL7 standard language for an XML-based representing medical knowledge as logic statements. Problems related to execution of ArdenML are the curly braces problem and the compiler problem, which may be resolved by the introduction of XML-based techniques. This study was aimed at develop an ArdenML based XSLT conforming to HL7 standard. Methods: There were several structural differences in three approaches: Arden Syntax is category; ArdenML is tag; and Visual Basic is comment, function, events, class, and procedure. XSLT was developed by analyzing three approaches. Based on XSLT, ArdenML was translated into Visual Basic and the interface module was developed to allow communicating with legacy systems. Results: Most of the CDSS are independently developed and run, to require reentry of rules. But this system makes it easier to interface with the existing systems simply by translating or compiling rules. Two hundred sixty five of the 285 rules were showed the success after compiling, and 6 errors for the operater query, 14 warnings related to data type were resolved by data re-modeling. Conclusions: ArdenML has been converted into executable files using standard transforms written in the eXtensible Stylesheet Language Transformation (XSLT) in visual basic language. Development of Arden Syntax compiler requires not only a lot of time and efforts, but also reorganization of existing systems according to the MLM. It also has curly brace problem and problem of requiring additional compiler.

Propensity score model 구축에서 상관성을 고려한 변수선택

Objectives: In the covariate selection for propensity score model (PSM), including all the covariates that can be observed has been recommended. However, there are problems that appear multi collinearity and do not obtain the matching number needed using over fitted propensity score model. In this study, we studied the method of variable selection for PSM considering the correlations between covariates. Methods: All the covariates were classified according to the relation with treatment and outcome and generated considering the correlations each other. We examined the odds ratio and MSE (mean squared error) of PSM and the matching number of simulated data. Results: When there are correlations among covariates included in PSM, the matching number decreased as the correlation of covariates was stronger. Also, the larger the strength of correlation among covariates was, the smaller MSE was and the matching number was. Conclusions: When including covariates in PSM, we found that it is more efficient to examine the correlation of covariates, treatment variable, and outcome variable than using all the covariates observed.

임상간호사의 의도적 간호순회가 환자의 간호요구와 간호서비스 만족에 미치는 영향

Objectives: This study aimed to examine the effects of intentional nursing rounds on patients’ needs and satisfaction. Methods: A non-equivalent control group non-synchronized quasi-experimental design was used. Adult inpatients were recruited from two medical/surgical units at a general hospital in Korea. While the control group (n=37) received standard care, the experimental group (n=39) received structured nursing rounds every two hours during the day and every four hours at night. For data collection, observation logs and self-report questionnaires were used. The collected data were analyzed using Chi-square, independent t-test, paired t-test and repeated measured ANOVA. Results: As time passed, the frequency of patients’ needs for nursing care was significantly reduced in the experimental group. The significant differences between two groups began to emerge from post-intervention day 4. Patient satisfaction on nursing services was significantly increased in the experimental group. In addition, the mean difference between pretest and posttest was significantly larger in the experimental group than in the control group. Conclusions: Systematic and purposeful nursing rounds were effective in addressing patients’ basic needs and in improving patient satisfaction with hospital nursing services. However, more research is needed to determine the influence of intentional nursing rounds on patient safety and nurse satisfaction.

재가 장애인의 지역사회중심 재활서비스 이용실태와 우울에 관한 연구

Objectives: This study was conducted to investigate the utilization and its determinants of community- based rehabilitation services and influencing factors on depression of people with disabilities. Methods: From November 3 to November 7. 2014, we interviewed 102 disabled people visited the community health center. The subjects completed questionnaires about the utilization of rehabilitation services, general characteristics and disability characteristics. Depression was measured using the CES-D. Descriptive statistics, t-test, ANOVA were used for statistics. Results: About 98.0% of subjects experienced CBR Services. 60.8% of subjects are using the CBR services every day. The two most common reasons for using the services were “Good to be effective for the service” and “Low price of service.” The facilities that the disabled people were currently using, in the order of most used, were Community health center, general hospitals or clinics, rehabilitation centers, and oriental medicine clinics. 93.1% of subjects responded they would continue to use the community-based rehabilitation services. Among the subjects 53.9% were found to be on the level of depressed state. Depression significantly is related with use of prosthetics (p=0.025) and satisfactory program of CBR service (p=0.024). Conclusions: Since the community-based rehabilitation services (CBR) for the disabled is being operated centering around the supplier’s side, it fails short of supply in terms of quality and quantity. And the real need of the disabled is not very well reflected. Therefore, more effectively operated CBR for the disabled, service facilities should be expanded and effective delivery system should be reorganized responding to their needs.

간호학과 학생들의 무례한 언행의 경험에 영향을 미치는 요인

Objectives: Incivility in cyber world has been considered as a negative gain of modern society. The present study explored cyber incivility of nursing students and the possible predictors. Methods: Participants (122 nursing students; 94 females and 19 males) indicated their gender, age and year, and then responded to the questionnaire including the cyber incivility, childhood abuse, behavioral activation and inhibition system, and emotional intelligence. Results: Nursing students showed low level of experiencing incivility during cyber interaction though social network service. Active cyber incivility was associated with gender, EQ, and drive. Specifically, male students, low level of EQ, and high level of drive related with high level of active incivility. Passive cyber incivility was associated with drive negatively. Conclusions: Considering clinical practicing and homogeneity of career development of nursing students, more attention and educational interests need to be focussed on their interaction. As far as cyber or internet communication concerned, strategy for enhancing emotional intelligence should be implemented.
