간호사가 지각한 병원조직의 내부마케팅 수행도, 감정노동, 직장-가정 갈등이 이직의도에 미치는 영향

Purpose: This study was done to identify internal marketing activity, emotional labor and work-family conflict that influence turn-over in hospital nurses. Methods: Participants were 513 nurses from 2 general hospitals in Chung-nam and Gyeonggi Provinces, South Korea. Data were collected from September 1 to 30, 2011 using self-report questionnaires. Stepwise multiple regression was used to identify association between variables. Results: The mean score for internal marketing activity was 94.7±14.7, emotional labor, 31.4±5.5, work-family conflict, 19.6±4.1, and turn-over, 13.7±3.4. After adjusting for general characteristics, stepwise multiple regression showed that work-family conflict (β=.20, p<.001), marketing activity (β=-.21, p<.001), and emotional labor (β=.22, p<.001) were associated with turn-over. These predictors accounted for 33% of variance in turn-over. Conclusion: The results of this study show that work-family conflict was the most influential predictor indicating a need for the development of strategies to reduce work-family conflict and emotional labor. At the same time internal marketing activity should be supported in the hospital by analyzing the needs for nurses. Also career development programs and mentor systems will be useful for effective manpower management and quality improvement of nursing services.

Purpose: This study was done to construct a hypothetical model to explain the factors influencing career commitment of nurses. Methods: Three factors (individual internal-, job- and career-characteristics) and 8 sub-factors were measured with Likert scales. Data collection was conducted from Jan. 11 to 31, 2011 using self-report questionnaires. Participants (738 nurses) were selected from 6 hospitals in 2 metropolitan cities of South Korea. Collected data were analyzed using SPSS Window 16.0 and Amos 19.0. Results: 1) Among the 3 factors, career commitment was most strongly influenced by career-characteristics. 2) Career commitment was influenced by organizational-career-management but not by personal-career-management. 3) Career commitment was influenced by individual internal-characteristics but not by job-characteristics. 4) Of the 8 sub-factors, career commitment was most strongly influenced by desire-for-growth but not by self-efficiency. Conclusion: The results of this study indicate that in order to improve career commitment of nurses it is necessary to implement custom-made career development policies considering individual characteristics in the organizational level.

간호요구 정도에 의한 신생아중환자 분류도구의 타당도 및 신뢰도 검증

Purpose: This study was done to verify validity and reliability of a neonatal patient classification system (NeoPCS-1). Methods: An expert group of 8 nurse managers and 40 nurses from 8 Neonatal Intensive Care Units in Korea, verified content validity of the measurement using item level content validity index (I-CVI). The participants were nurses caring for 469 neonates. Data were collected from November 11 to December 14, 2011 and analyzed using descriptive statistics, ANOVA, intraclass correlation coefficient, and K-cluster analysis with PASW 18.0 program. Results: Nursing domains and activities included 8 items with 91 activities. I-CVI was above .80 in all areas. Interrater reliability was significant between two raters (r=.95, p<.001). Classification scores for participants according to patient types and nurses` intuition were significantly higher for the following patients; gestational age ( 29 weeks), body weight (<1,000 gm), and transfer from hospital. Six groups were classified using cluster analysis method based on nursing needs. Patient classification scores were significantly different for the groups. Conclusion: These results show adequate validity and reliability for the NeoPCS-1 based on nursing needs. Study is needed to refine the measurement and develop index scores to estimate number of nurses needed for adequate neonatal care.

Purpose: The purpose of this study was to evaluate the effects of a home-based video exercise program on cancer-related fatigue, physiological and psychological status in patients with colon and rectal cancers undergoing chemotherapy. Methods: The study design was a non-equivalent control group non-synchronized design. Data were collected from patients with colo-rectal cancers in Yonsei cancer center from July 5th to October 31st in 2011. There were 40 participants; 20 in the experimental group and 20 in the control group. The structured questionnaire was used to measure fatigue, physical function and emotional status. Data were analyzed using SPSS 18.0 and a chi-squre test, Fisher`s exact test, Mann-Whitney U test and Wilcoxon signed- rank test were conducted to examine the homogeneity and the research hypotheses. Results: There was a statistically significant difference in White Blood Cell count in the experimental group compared with that of the control group. The exercise group showed a slight decrease of White Blood Cell count compared with that of the control group after 4 week program (z=-2.935, p=.003). However, there were no significant differences in fatigue, physiological and psychological status between the two groups. Conclusion: In this study, the developed video exercise program was effective in markedly slightly decreasing White Blood Cell count in patients with colo-rectal cancers undergoing chemotherapy. Therefore, utilizing the video exercise program can be an useful method to promote health among patients with cancer in clinical practice.

시뮬레이션기반 감염관리교육이 중환자실 간호사의 감염 관리에 대한 인식도, 임상 수행도, 자기효능감에 미치는 영향

Purpose: This study was conducted to examine the effects of simulation-based infection control training on the ICU nurses` perception, clinical performance, and self-efficacy of infection control. Methods: Thirty-eight nurses were assigned into two groups using a career stratified randomization. In the experimental group, the subjects received a simulation-based infection control training, whereas the control group participated in a conventional lecture-based training. Two weeks after the completion of the training sessions, the participants were evaluated for perception, clinical performance, and self-efficacy regarding the infection control. Results: The experimental group that received simulation-based infection control training showed an improvement in perceiving the infection control compared to that of the control group, but the difference was not statistically significant. In terms of the clinical performance, the experimental group and the control group scored 26.05± 3.22 and 18.53±3.37 points respectively, demonstrating a statistical significance (p<.001). There was no significant difference between the two groups in regards to the self-efficacy. Conclusion: The developed simulation-based infection control training showed positive effects in improving clinical performance of infection control over conventional lecture-based training, confirming that a simulation-based training is an effective method in advancing the practical performance of ICU nurses.

아로마 흡입요법이 대장내시경 검사 대상자의 오심, 구토, 복부 불편감, 순응도 및 대장 정결도에 미치는 효과

Purpose: The purpose of this study was to identify the effects of aroma inhalation therapy on nausea, vomiting, abdominal discomfort, compliance, and colon cleansing for patients having a colon lavage solution before colonoscopy. Methods: This study was performed using a nonequivalent control group post-test design. The study was conducted from 20 Jun to 29 Aug, 2011. The participants were 152 patients (experimental group: 77, control group: 75) who underwent colonoscopy at A hospital, Seoul. Results: The experimental group with the aroma inhalation therapy reported significantly lower nausea and vomiting than did the control group. There were no significant differences in abdominal discomfort, compliance, and colon cleansing between experimental and control group. Conclusion: Use of an aroma inhalation therapy was efficient in nausea and vomiting among patients having a colon lavage solution before colonoscopy. Therefore, aroma inhalation therapy is recommended as an effective nursing intervention for relieving nausea and vomiting among patients having a lavage solution before colonoscopy.

역기능적 신념과 정서조절 양식이 간호사의 문제해결 능력에 미치는 영향

Purpose: The purpose of this study was to explore influencing factors of dysfunctional beliefs and emotion regulation strategy for nurses` problem solving ability. Methods: This study was a cross-sectional design with a sample of 745 nurses from 1 university hospital located in Gyeonggido. The scales were Dysfunctional Beliefs Test (70 items), Emotion Regulation Strategy Questionnaire (25 items) and Social Problem Solving Inventory (52 items). The data were analyzed using SPSS 17.0 employing ANOVA, pearson correlation coefficients and multiple regression analysis. Results: The mean score for problem solving ability was 11.26 points. Influencing factors for nurses` problem solving ability were identified as ``active regulation style`` in emotion regulation strategy and ``negative concept of social self`` in dysfunctional beliefs. Conclusion: It is plausible to assume that dysfunctional beliefs which are vulnerability factors in cognitive variables and emotion regulation strategy affect nurses` problem solving ability.

임상간호사의 조직사회화와 조직시민행동 간의 관계

Purpose: This study is to determine the patterns of relationship between organizational socialization (OS) and organizational citizenship behavior (OCB) among nurses working in a general hospital setting by examining whether organizational adaptation through OS leads to positive behavioral changes, which may be helpful to improve organizational performances. Methods: The data were collected from 494 nurses in five general hospitals using a structured questionnaire, and they were analyzed by conducting canonical correlation analysis using SPSS 12.0. Results: The analysis of the relationship patterns between OS and OCB revealed that two significant canonical correlations between the two groups of nurses with less than 5 years of working experience (LT) vs. more than 5 years of experience (MT), respectively. Thus, the complicated correlations between OS and OCB were identified. The variables that contributed to each canonical function most were ``politics``, ``organizational goals and values`` in the LT group, and ``organizational goals and values`` and ``language`` in the MT group. The notable point in the relation between OS and OCB in our study was negative contribution of ``sportsmanship`` among the OCB variables in the MT group. This is thought to be the negative effect of OS. Conclusion: More effective workforce management could be achieved by adopting a differentiated approach according to work experience. To develop a better program to enhance OS, more careful attention to OCB of experienced nurses should be paid to minimize a negative effect on organization while rewarding individuals or departments for a positive organizational behavior.

집단재활체조 프로그램이 뇌졸중 환자의 우울과 재활동기에 미치는 효과

Purpose: The objective of this study was to investigate the effects of a group rehabilitation exercise program for stroke patients on their depression and motivation of rehabilitation. Methods: The design of this study was the nonequivalent control group non-synchronized quasi-experimental design. Data were collected from June through August in, 2012 in a rehabilitation hospital located in the C city. A total of 63 patients participated in the study [experimental group (n=32) and control group (n=31)]. The data were analyzed using x2-test and t-test. Results: The depression in the experimental group was lower than the control group (t=2.446, p=.017). The motivation of rehabilitation in the experimental group was higher than the control group (t=-4.90, p<0.001). Conclusion: Based on the results, the group rehabilitation exercise program for stroke patients is useful to decrease their depression and improve their motivation of rehabilitation. The program is considered to be an effective nursing intervention for decreasing the stroke patients` depression and improving their motivation of rehabilitation.

통원 수술환자의 안전한 귀가 결정을 위한 퇴실기준에 관한 연구

Purpose: This study was conducted to compare three discharge criteria; 1) discharge criteria of S Hospital determined by nurses, 2) discharge readiness determined by patients, and 3) the Modified Post-Anesthetic Discharge Scoring System (MPADSS). The usefulness of MPADSS as a discharge criteria for the patients`safe return to home after ambulatory surgery was also evaluated. Methods: A total of 370 day surgery cases were investigated. The MPADSS was employed in every 30 min. in parallel with discharge readiness assessment by nurses and patients. The percentage of the patients who were categorized as being ready to discharge were compared according to three discharge criteria. Results: The percentage of patients scored to be as MPADSS > 9 in 30 min, 60 min, 90 min were 96.5%, 99.5%, 100% respectively. Whereas 11.1%, 44.3%, 71.1% of patients rated themselves as being ready to discharge and 2.7%, 23.5%, 54.3% of patients actually discharged by nurses according to discharge criteria of S Hospital. Conclusion: Nurses tend to keep patients longer in the hospital when compared to the patient`s own assessment about their readiness to home and to that of MPADSS. Faster discharge in the evening than day time suggests patient discharge can be influenced by nursing factors. This brings out the importance of scoring system to determine the safe discharge. The MPADSS could be a useful tool in evaluating patients for safe discharge.
