간호사의 팔로워십 유형, 자아탄력성이 직무배태성에 미치는 영향

Purpose: The aim of this study was to investigate and identify the influence of nurses' followership types and ego-resilience on job embeddedness. Methods: Self-reported questionnaires were distributed to 546 nurses working at hospitals with 400 to 700 beds in Seoul and Gangwon Province, and 520 sincere questionnaires were analyzed using SPSS / WIN ver 23.0 program. Results: The job embeddedness, ego-resilience and followership types of the participants showed significant correlation with each other. In hierarchical multiple regression analysis, nurses' followership types and ego-resilience were identified as predictors of job embeddedness (Adj. R2=0.34, p<.001). Conclusion: Findings of this study indicate that nurses need to develop their followership and ego-resilience to increase job embeddedness. Future studies should explore ways to improve followership and ego-resilience.

Purpose: The purpose of this study was to identify the influence of nursing organization culture on resilience of new nurses. Methods: Participants were 1,033 new nurses with careers of less than 12 months and who were from 43 general hospitals located in Seoul, Incheon, other metropolitan cities and provinces in Korea. The data were analyzed using t-test, ANOVA, Pearson correlation coefficient and multiple regression with SPSS/WIN 24.0 program. Results: Relation-oriented culture was scored highest for perception of organizational culture by new nurses (3.35), followed by hierarchy-oriented culture (3.19), innovation-oriented culture (3.12), and task-oriented culture (2.73). The score for resilience of the new nurses was 3.49, and the first subcategories were interpersonal relationship ability 3.66, self-positivity 3.45, and self-control ability 3.36. Relation-oriented culture (β=.30, p<.001) was the most significant factor influencing resilience of new nurses, followed by innovation- oriented culture (β=.14, p=.001), hierarchy-oriented culture (β=.12, p<.001), task-oriented culture (β=-.10, p=.003) in that order (R²(adj. R²)=.16, F=47.04, p<.001). Conclusion: Findings show that for improved resilience in new nurses it is necessary to develop relation-oriented culture to provide a cooperative work environment( friendly interpersonal relation and dialogue) for new nurses and to encourage involvement of these members in the hospital culture.

Purpose: This study was conducted to identify the effects of educational video program delivered using Tablet PC on physical discomfort, uncertainty, state anxiety, and nursing education satisfaction among early gastric cancer patients undergoing endoscopic submucosal dissection. Methods: The study design was nonequivalent control group pretest-posttest design. The subjects were 60 patients who were hospitalized to undergo endoscopic submucosal dissection. The experimental group watched educational video using Tablet PC (n=30) and the control group received only the usual education (n=30). The collected data were analyzed using independent t-test to examine study hypothesis. Results: The level of physical discomfort (t=3.05, p=.003) and nursing education satisfaction (t=-2.20, p=.032) in the experimental group were significantly different from that of the control group. However, the level of uncertainty (t=-0.82, p=.418) and state anxiety (t=-1.69, p=.097) in the experimental group were not different from that of the control group. Conclusion: The study findings confirm that the educational video program delivered using Tablet PC as an effective intervention alleviating physical discomfort and improving satisfaction regarding nursing education among early gastric cancer patients undergoing endoscopic submucosal dissection. Based on the findings, we believe that the educational video program can be helpful in decreaseing physical discomfort, and it also can be utilized to improve nursing education satisfaction.

Purpose: The purpose of this study was to investigate the reality of workplace violence experienced by emergency nurses and the relationship of violence response to professional quality of life. Methods: The participants in this study were 899 emergency nurses from Korea nationwide. Data were obtained through an online survey done during October, 2017. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient. Results: Of the emergency nurses, 72.1% recognized that workplace violence is serious. Experience with workplace violence correlated positively with violence reaction (r=.32, p<.001), burnout (r-.20, p<.001) and secondary trauma (r=.22, p<.001). Also, reaction to violence was positively correlated with burnout (r=.28, p<.001) and secondary trauma (r=.56, p<.001). Conclusion: Findings indicate that the diverse workplace violence experienced by emergency nurses decreases their professional quality of life. Further study is needed to develop solutions to the problem of workplace violence in emergency settings.

Purpose: The aim of this study was to examine effectiveness of 12-hour shifts for nurses compared to 8-hour shifts for the variables: Work-Life Balance, fatigue and work errors. Methods: In 2014, an opportunity to choose a 12-hour shift duty was given to a group of 8-hour shift nurses. In 2016, two years after this change, this study was done to compare the two groups. Data were collected using questionnaires. Data were sampled by a matching method with propensity score matching (PSM). The participants were 128 nurses: 64 nurses on 12-hour shifts and 64 nurses on 8-hour shifts. The comparison was analyzed using x2 test, t-test. Results: The nurses on 12-hour shifts showed higher scores for Work-Life Balance (3.37) than the groups on 8-hour shifts (2.99)(p=.018) whereas were no statistical differences between the groups for fatigue (p=.132) or work errors (p=.703). Conclusion: The Work-Life Balance scores for nurses who chose the 12-hour shift shows an enhancement without an increase in fatigue or work errors.

Purpose: The purpose of this study was to explore the relationships among workplace bullying, organizational commitment, and turnover intention of nurses. Methods: A cross-sectional study was conducted. Data were collected using a structured questionnaire from nurses at five public hospitals (N=190). Results: The experience of bullying was 31.29 points (range 16-64) on average: Work-related bullying was the highest, followed by verbal/non verbal bullying and external threats. The consequences of bullying were 30.54 points (range 13-52) on average: The physical/psychological withdrawal was the highest, followed by increasing distrust and the poor quality of patient care. The experiences of bullying had positive correlations with the consequences of bullying (r=.55, p<.001) and the turnover intention (r=.27, p<.001), and had a negative correlation with organizational commitment (r=-.28, p<.001). The consequences of bullying had a positive correlation with the turnover intention (r=.52, p<.001), and had a negative correlation with organizational commitment (r=-.49, p<.001). Organizational commitment showed a negative correlation with the turnover intention (r=.63, p<.001). Conclusion: The findings of this study suggests the needs of a regular monitoring system and organizational management for workplace bullying, which helps nurses to commit to their organization and to be retained in a workplace.

Purpose: The purpose of this study was to compare the differences in compassion fatigue, compassion satisfaction and burnout between nurses working at comprehensive nursing care unit and general ward. Methods: The subjects were 35 nurses in general ward and 42 nurses in the Comprehensive Nursing Care unit in one hospital. Measurement instrument included the Stamm’s professional quality of life (ProQOL) version 5-Korean. Descriptive statistics, t-test, paired t-test, and ANCOVA were used to analyze the data. Results: Nurses working at the Comprehensive nursing care unit demonstrated significantly lower compassion fatigue (F=17.00, p<.001), higher compassion satisfaction (F=14.39, p<.001), and lower levels of burnout (F=40.07, p<.001) than control group. Conclusion: Compassion fatigue and burnout were lower and compassion satisfaction was higher among nurses working at comprehensive nursing unit than general ward. In order to improve quality of the comprehensive nursing care services, there is a need to be concerned with the nurse's compassion fatigue, compassion satisfaction, and burnout.

Purpose: This study aimed to investigate the current statistics of professional medical support staffs(PMSSs) working in general hospitals with less than 500 beds. Methods: This study was conducted on 35 general hospitals with less than 500 beds from September 11th to October 27th, 2017. Results: Four hundred fifty one PMSSs were currently providing medical support. The number of clinical nurse experts was the highest among the roles, followed by Physician Assistants(PA) and Advanced Practice Nurses. The mean job satisfaction score was 3.07 out of 5 . In the case of PA group, most of the delegated prescriptions were performed, however the delegated roles were not much documented in written format. The paucity of documentation requires a development of a committee for PMSSs, including a development of selection criteria and a scope of practice in each institution. Conclusion: The results of this study suggested the composition of a committee for PMSSs in the medical institutions and renaming the specified titles of PMSSs.

Purpose: This purpose of this study was to develop evidence-based practice guideline for isolation in health care settings to prevent transmission of infectious diseases utilizing guideline adaption process. Methods: The process of guideline adaptation was performed according to the Korean hospital nurses association’s guideline adaptation manual which consisted of three main phases, 9 modules, and 24 steps. Results: The adapted isolation guideline consisted of introduction, overview of isolation guideline, summary of recommendations, recommendations, references, and appendices. The guideline includes 224 recommendations in 4 sections which are organizational administration, standard precautions, transmission-based precautions, and education/ counselling. Conclusion: The adapted isolation guideline is recommended to be disseminated and utilized by nurses and clinicians nationwide to improve the isolation practices for infected or colonized patients with communicable diseases and to decrease the transmission of infections in the healthcare settings.

Purpose: This study compared the temporal artery temperature (TAT) measured by infrared temporal artery thermometers to the axillary temperature (AT) measured by standard mercury-in-glass thermometers, and evaluated accuracy of the TAT measurement for clinical practice. Methods: A total of 247 adult inpatients in general wards in a tertiary medical center located in Seoul participated in the study. The TAT was measured within one minute after the AT measurement. Data were analyzed using descriptive statistics, paired t-test, Pearson correlation coefficient, linear regression, and the Bland-Altman plot. Results: There was a significant difference in mean temperature between AT and TAT, 36.89℃ (SD=0.70) versus 37.35℃ (SD=0.72). The Bland-Altman plots demonstrated the difference between the AT and TAT as -1.29 to +0.33. The specificity and sensitivity of the TAT in detecting fever were high. The positive predictive values were 57.5% and 71.0% when the AT were higher than 38.0℃ and the TAT fever cutoff levels were 38.0℃ and 38.3℃ respectively. Conclusion: TAT and AT were highly correlated and agreeable, indicating that TAT is as accurate as AT. The findings suggested that TAT measurement can be used in clinical practice. For accurate communication between medical personnel, medical institutions need to provide guidelines for temperature measurement, especially for the use of thermometer and measurement sites.
